The marquee injuries keep piling up, and if your fantasy roster is starting to look more like an injury report, I feel ya. But by reading this, you're already setting yourself up for success and getting ahead of the latest injury news. It's a long season. Hang in there.
WR Puka Nacua, LA Rams | 1:00 PM EST Kickoff
After a frustrating two-week absence, Puka Nacua is back for Week 4. Historically, WRs returning from groin strains see a moderate decline in fantasy production in their first game back, with roughly 25% falling outside normal variance. For Nacua, that could mean closer to 12-14 fantasy points rather than his usual 20-point ceiling. This aligns with the latest reporting from Tom Pelissero of The Ringer and Netflix, who has indicated Nacua could be on a snap count in Week 4.
The good news is that these declines are typically short-lived, with no evidence of significant long-term effects. Regardless of the playing time concerns, I'm starting Nacua in every format. You don't bench your first-round pick when they are going to be on the field.
WR Devonta Smith, Philadelphia | 1:00 PM EST Kickoff
DeVonta Smith played 100% of the snaps Monday night but was added to the injury report with a hamstring issue and has been ruled out for Week 4. Given that he finished the game, it's possible this is more tightness or soreness that surfaced afterward rather than a significant aggravation. Philadelphia may simply be taking a cautious approach with an injury Smith also dealt with during the preseason. Assuming no setbacks, I wouldn't rule out a Week 5 return.
WR Justin Jefferson, Minnesota | 4:05 PM EST Kickoff
Justin Jefferson did not practice all week and was officially ruled out for Week 4 with an eversion/medial ankle sprain. While a Week 5 return is possible, I'm growing more skeptical given that Jefferson has yet to practice in any capacity. We've also recently seen players attempt to return from similar injuries only to aggravate them and land on IR. With Minnesota's bye coming in Week 6, I wouldn't be surprised if the Vikings take the cautious approach and hold Jefferson out until Week 7.
WR Mike Evans, San Francisco | 4:25 PM EST Kickoff
Mike Evans and the 49ers are not ruling out a Week 4 return, and his injury history gives me some optimism that he plays. Every time Evans has appeared on the final injury report, he has played, including once when listed as doubtful, a designation that results in a player sitting 94% of the time in my database. Evans also carries a 90th-percentile Recovery Score, meaning he has historically recovered from injuries faster than expected compared with other NFL players. If active, the rib injury could limit his snaps or workload, but I still expect Evans to be a focal point when he's on the field. The biggest concern for fantasy managers is the 4:25 p.m. ET kickoff. If you plan to wait on Evans, make sure you have a late-window pivot available.
