Week 7 is shaping up to be a game of medical mystery, and unfortunately, your fantasy lineup is the crime scene. Puka Nacua's calf has everyone on edge, Lamar Jackson's ankle finally has a diagnosis, and DeVonta Smith is doing his best to make Monday night as stressful as possible. Some of these players are trending in the right direction, while others are one questionable designation away from ruining your Sunday. Fortunately, we've got injury data, practice trends, and historical recovery patterns to help separate legitimate concerns from the usual NFL smoke screens. Consider this your respirator.

WR Puka Nacua | 4:25 PM EST Kickoff

Of all the players discussed this week, Puka Nacua has the shakiest outlook heading into Week 7. Nacua was a late addition to the injury report with a calf injury, and despite initial optimism, NFL Network's Ian Rapoport described his availability as "very much up in the air." I still lean toward Nacua playing based on his practice trends and history of playing through injuries, but his previous groin/core issues and concerns about a potential sports hernia add another layer of uncertainty. Calf injuries have also historically been problematic for WRs, often resulting in reduced fantasy production and usage upon return. If Nacua is active, I'm still starting him in fantasy lineups, but his floor is considerably lower than usual.

QB Lamar Jackson | 1:00  PM EST Kickoff

Lamar Jackson has missed the past two weeks with an ankle injury, which was finally confirmed Friday as a high ankle sprain. This diagnosis helps explain his extended absence and raises concerns about his availability this week. High ankle sprains typically carry a 3-4 week recovery timeline, and Jackson's historical Recovery Score ranks in the bottom 25th percentile, suggesting he tends to recover more slowly than positional averages. Given how much Jackson relies on his mobility to extend plays and generate fantasy production, returning prematurely could also limit his effectiveness. I lean toward Jackson missing another week and would like to see him log at least one full practice before feeling confident about his return.

RB Jordan Mason | 1:00  PM EST Kickoff

Jordan Mason is expected to return from IR after missing four games (five weeks including the bye) with a thumb injury. Despite practicing in full all week, the Vikings have listed him as questionable for Week 7. Historically, thumb and hand injuries have not been associated with significant declines in fantasy production or usage for RBs upon return. While ball security might be a concern, I think worrying about that would be overanalyzing the situation. Mason handled 44% of Minnesota's rushing attempts in Week 1, and a similar workload in this favorable matchup with the Colts should translate into fantasy points. I expect Mason to play, and if he's active, I'm putting him in my lineup.

QB Baker Mayfield | 1:00 PM EST Kickoff

Baker Mayfield has been pushing to return, and based on practice trends and recent reports, it appears he will be back this week. Historically, quarterbacks returning from hand injuries tend to experience some decline in performance, particularly with passing accuracy. Before his injury, Mayfield was completing roughly 63% of his passes, already a decline from his 2023 and 2024 numbers when he was a top-10 fantasy quarterback. Given these trends, I would not be surprised if Mayfield experiences some inconsistency as he works his way back. With Emeka Egbuka already ruled out, there are additional reasons to temper expectations for Mayfield's fantasy outlook this week.

WR DeVonta Smith | 8:15 PM EST Kickoff - MONDAY

DeVonta Smith continues to deal with a hamstring injury that dates back to the preseason and previously sidelined him for Weeks 4 and 5. Although Smith returned in Week 6, his lack of practice participation heading into Monday night raises concerns about his availability. Philadelphia reporter Jeff McLane suggested Smith was trending toward missing Week 7, although a few days remain before kickoff. I would be surprised if Smith returned from his initial hamstring injury only to suffer another setback so quickly, but it's certainly possible. In my opinion, planned rest for Smith was always on the menu. I remain cautiously optimistic about his chances of playing, with Saturday's practice participation likely providing more clarity.
